July 30, 2024: School Physicals, Sports Physicals, and Immunization Clinic 8:30 AM -12:30 PM and 1:30 PM - 7:30 PM at JCHS

• Open to all students

• Physicals are $25 cash or check (Insurance will not be billed)

• Immunization Clinic - will be billed through private insurance and Medicaid, or $20 per shot for uninsured.

• Signed Consent Note: If a Parent/Guardian is not present at the time of the physical, please send a signed permission slip giving consent for your child to receive a physical.
